The forms below are provided for the convenience of parents/carers and students, allowing forms to be submitted electronically at any time. Forms can also be accessed through the School Bytes Parent Portal. Submitted forms will be automatically directed to the appropriate staff member, and you will receive an acknowledgement with a copy of your completed form.
Online submission is our preferred method. If you are unable to complete a form online, you may download and complete the form and email it to the school, or print and submit the completed form to the Front Office.
F.01 Change of Contact Details
To update contact details for parents, students and emergency contacts.
F.02 Authority to Exchange Information
Provides consent for the school counsellor to exchange information about a student's wellbeing with a nominated person.
F.03 Extracurricular CAPA Activities
To express interest in a student participating in extracurricular Creative and Performing Arts (CAPA) activities.
F.04 Senior Agreement (for Year 11 and 12 students only)
For parents and students, to acknowledge and agree to the expectations and requirements.
F.05 Application for exemption from attendance: 99 days or less
To apply for an exemption from school attendance for up to 99 days due to employment in the entertainment industry or participation in elite arts or sporting events.
F.06 Application for Extended Leave - Travel
To apply for extended leave from school for travel, holidays or bereavement.
To notify the school that a student is leaving and provide details of their next school or destination.
F.08 SRE and SEE Participation
Provides/removes parent or guarding permission for a student to participate in Special Religious Education classes.
Provides/removes parent or guardian permission for a student to participate in Voluntary Student Activities of a Religious Nature.
F.10 Parent Transport of Students To/From a School Endorsed Activity in a Private Vehicle
For parents of guardians who are transporting students to or from a school-endorsed activity in a private vehicle.
To apply for equity assistance for a Bring Your Own Device (BYOD).
F.12 Student Assistance Request
To request financial assistance for school related costs such as subject fees, uniforms and excursions.
For parents and carers to provide permission for the school to publish photographs, videos or other information featuring their child in school communications and publications.
F.14 Extra-curricular Achievements
For parents and carers to tell us about their child’s sporting or extra-curricular achievements at state, national or international level so we can celebrate their success in our school community.
